Making a referral

Referrals to the service should be made using the standardised referral form here and emailed to Dhcft.perinatalcmht@nhs.net 
 

Things to consider in your referral:

  • Does the women / birthing person consent to the referral 
  • Please provide information regarding the date of the loss, type of loss (e.g. miscarriage, stillbirth) and a summary of what happened. 
  • Has it been at least one month since the loss 
  • Please include any history of previous obstetric loss 
  • Details about current mental health (e.g. low mood, anxiety, sleep disturbance, irritability, loss of appetite, hopelessness, blame / guilt, intrusive thoughts, flashbacks, nightmares) 
  • Any current medication being prescribed (antidepressants, mood stabilisers, antipsychotics), if not would the person benefit from a medication review by their GP. 
  • Any risk in terms of self-harm or thoughts about not wanting to be here. 
  • If the person is experiencing suicidal thoughts with intent to harm themselves, the referral should be sent to the Crisis Team. 
  • Any safeguarding concerns around other children in the family home being cared for by the person being referred.
